This is the most common change order in flooring work. You don't know the subfloor condition until the old flooring comes up. Water damage, rot, uneven concrete, or deteriorated plywood can add $2-$5/sq ft in repair costs. A good proposal includes a conditional line item: 'Subfloor repair, if needed, at $X/sq ft, to be confirmed after tearout.' This way the client knows the possibility exists and the price is agreed before the surprise.
Most installers offer furniture moving as a service ($100-$300 per room depending on the amount). Some include it in the price, some charge separately. Either way, it should be on the proposal. Items you're responsible for regardless: electronics, breakables, items inside furniture (empty the dresser drawers), and anything in closets. The installer won't disassemble beds or disconnect appliances unless that's specifically in the scope.
